Our Programs
Housing:
Mesa & Gray House:
In
these programs, clients seek to achieve their highest level of
independence in an environment that is supportive and consistent. They may remain at these
houses as long as they are receiving benefit from the services provided
and they are able to abide by the rules.
Hudson & Jolly House: Provides a residential
treatment program in a group home setting for clients that suffer from
chronic mental illness and addictive illnesses. In these programs, a
client is encouraged to work through an individualized program and
achieve greater independence in 18 months – 2 years. The
ultimate goal is graduation to more independent living at that time.
Shelter Plus Care: Provides
an unsupervised, subsidized apartment program with weekly program and
participation requirements. Active
case management is required.
Concha Apartments: Unsupervised,
subsidized rental apartments managed by TLS.
Case management is not required but may be
recommended. Some
single room apartments and some double occupancy apartments. Unlimited length of stay.
Other Critical Services:
Case Management: Case
Management offers individualized treatment by a client advocate. Case Managers identifies
needed support systems in the community such as physicians,
psychiatrists, and families, and then works with the individual to
access those support systems. A
yearly treatment plan is developed with the consumer to identify
rehabilitation goals and is updated once a quarter to monitor progress. Required by anyone in the
Group Homes or Shelter Plus Care; also available for those living
independently.
Psychosocial Rehabilitation (PSR or IPSR): Individual
and group rehabilitation programs designed to enhance life management
and interpersonal skills, teach symptom and anger management, and
develop creative and expressive abilities while increasing
socialization, community and peer supports.
Bridgeway Clubhouse:
Bridgeway Club is a Clubhouse program
based on the Fountain House Model that began in New York more
than 50 years ago. This program is an “intentional
community” based program that allows its members living with
the effects of mental illness to participate side-by-side with staff in
the running of the program. The primary goal of the clubhouse community
is to foster the concept of “shared ownership and
responsibility for the success of the program.”
Financial Management:
TLS offers financial management to clients who have
difficulty maintaining a budget on a limited income.
This service is required for all residents in the
Group Homes.
Therapy: Individual,
group, and family therapy sessions are available to TLS clients through
a team of counselors and social workers.
Therapists work with specific houses and programs so
that services may best be tailored to client needs.
