|Job Title||Community Support Worker|
Provide clinical case support to clients. In office and home visitation are required. Work as a member of the TLS Treatment Team as well as participate in individual and group supervision. Perform a variety of casework duties including assessment of client's psychosocial and multi-systemic needs; level of family functioning; formulation of clinical service/treatment plans; implementation of services to assigned clients; provide advocacy for clients as needed. Build and maintain positive working relationships with other agencies and institutions in the community to facilitate treatment. Documentation services provided to include: progress notes, treatment plans, releases of information, etc. Attend all TLS in-service and staff meetings. Maintain ongoing communication regarding treatment process with supervisor. Perform other duties as assigned by supervisor.
Bachelor's degree in social work, psychology, or related field and one (1) year post-bachelor’s experience in clinical case work. Valid driver’s license and vehicle. Excellent written and oral communication skills. Ability to work in a team as well as independently. Excellent time management skills. Ability to work non traditional hours. Ability to complete tasks in a timely manner.Negative T B Screening. Negative Alcohol & Drug Screening. Successful Criminal Background Check. Successful Clearance through the Employee Abuse Registry
